Join this expert lead webinar on transitional care in rheumatology, focused on the practical assessment and management of adolescents and young adults moving from paediatric to adult services. The session will cover how to approach consultations with 16–18-year-olds in clinic, principles of setting up and delivering an adolescent-friendly rheumatology service in a district general hospital setting, and common clinical scenarios involving juvenile-onset rheumatic disease presenting in young adulthood. The webinar will also highlight key transition frameworks, curriculum-relevant topics, and examination-focused themes to support day-to-day practice and Specialty Certificate Examination preparation.

Learning objectives:

• Understand the core principles of effective transition from paediatric to adult rheumatology services.
• Develop practical approaches to consulting with 16–18-year-olds in rheumatology clinic, including communication, confidentiality, autonomy, and family involvement.
• Recognise how adolescent-friendly rheumatology services and transition clinics can be structured in routine NHS practice.
• Review important curriculum- and SCE-relevant topics in adolescent rheumatology, including the management of juvenile idiopathic arthritis in young adults.
• Apply learning to common real-world clinical scenarios involving young people with chronic rheumatic disease.
